Goals & Objectives:
Upon completion of this course, pharmacists and pharmacy technicians will be able to:
• Discuss the prevalence of medication errors and their impact on patient safety.
• Recognize that medication misuse is a major threat to the safety of a patient.
• Identify options a pharmacist has to improve medication access for patients.
• List the different ways a pharmacist can provide health wellness services to their patients.
• Describe how pharmacists can help evaluate the health of their patient and make the proper referrals.
• Discuss the significance of assessing medication appropriateness prior to dispensing.
• Recognize the benefit of providing additional information and resources to patients.
• Name the methods available for improving medication adherence.
• Outline available programs to deliver medication therapy management services to patients.
• Identify the ways pharmacists help to make a patient’s transition of care seamless through proper medication management.
• Describe the value to patient’s safety from having pharmacists report adverse events.
• Recognize the advances pharmacists have made in increasing safety for opioid users.
• Discuss the reasons to design and maintain a proper Quality Improvement Program.
• Identify the responsibilities of the pharmacist in medication safety during Drug Clinical Trials.
• Define “Just Culture” and how it is directly linked to patient safety.
• Describe the Community/Ambulatory Care ISMP Medication Safety Alert! Newsletter and how it is improving medication error prevention.
